Mars is the twin planet of Terra, and one of many long-inhabited worlds near to the Cradle of Humanity. It is the home and power centre of the Adeptus Mechanicus, the ancient priesthood of the Machine God, and the heart of their galaxy-spanning monopoly on Imperial technology. Mars is peppered with towering hive cities, and there is very little of the planet's surface which remains untouched by the constructions of the Tech-priests. It is hinted that C'tan, the Void Dragon, may be dwelling under the surface of Mars. A piece of writing in the Necron codex, in which Abaddon the Despoiler is seen seeking information from a daemon, indicates the location of the C'Tan. The daemon hints that the Void Dragon is at Vaul's Moon. Vaul is the Eldar god of the forge. Since Mars is the greatest forgeworld in the galaxy, it can be assumed the Dragon may be there. There are also other pieces of backstory that provide similar evidence. This point is open to debate though and has not been confirmed by Games Workshop. It is also possible the Adeptus Mechanicus worship the ancient C'tan (although unknowingly) as they worship a figure called the Machine Spirit, which is possibly the Void Dragon, while the Omnissiah, the Machine Spirit made manifest, is the Emperor of Mankind himself. Therefore this leaves much debate in the topic, since in the same book from The Horus Heresy states that the Emperor in a past life defeated the dragon and imprisoned it, while it was recovering on Earth in the country of Libya. The planet is home to three Titan Legions, one of them the Legio Ignatum. The identity of the other two are classified.
Ophelia VII was an Imperial Cardinal World and the base for the secondary Adepta Sororitas convent, the Convent Prioris (According to Rick Priestley's Rogue Trader and second edition's Codex Imperialis the Prioris is located on Ophelia, while the Sanctorum is on Terra; Gav Thorpe's Sisters of Battle codex however reverses the locations). It is the base of the Ecclesiarchy's Synod Ministra.
Codexes are a type of lore entry that you can collect in-game - they can come from hidden glowing blue object in the open world, from defeating certain enemies, and from completing certain quests. Some codex entries can be found by all types of characters, while others are restricted to characters who are a certain class or faction.
Almost all codex entries are per-character, except for some of the Datacron entries, which are legacy-wide, and some Planet entries that you have earned a Datacron entry on will also be unlocked legacy-wide. The rest you will need to re-earn if you want them unlocked on your other characters.

How to get the Decimus's Rise codex: Awarded to the player during the Corellia planetary story mission "Making History". After entering the Decimus's Rise instance, the player should walk around a bit until the codex triggers. Going straight to Darth Decimus and out might cause the codex to be missed.
How to get the Master Arfan Ramos codex: Awarded to the player when they reach the security door inside the green jedi enclave, as part of the Corellia planetary story mission "The Battle of Corellia"
How to get the Corellian Defender-class Light Corvette (Knight/Consular) codex: As soon as you are given the mission to head to your ship for the first time, as part of the Jedi Knight\Consular storyline.

How to get the Darth Arho codex: Awarded after the player defeats Darth Arho as part of the Ilum storyline's final confrontation. (note: codex entry "Ilum Temple" gained when you enter the temple, just in case)
How to get the Iokath Remotes codex: From the Iokath Fractured Alliances storyline (not from the Chapter or dailies) from the quest where you get turned in to a droid iokath remote and and load the power cores - not a side quest, part of the main story (quest called The Lost Arsenal).
How to get the Pneumatic Trams codex: Unlocks during the Iokath storyline (not from the dailies) that takes place after Knights of the Eternal Throne. The codex is received after fighting the droid with a funny hat and electrical spiky balls, once you step on to the tram outside the door.
How to get the The Six Gods codex: Accept the Facing the Machine Gods quest from the the Iokath quests board, and then speak to the Scions, and when the conversation completes you will receive The Six Gods codex.

How to get the Zakuulan Allies [Sana-Rae] codex: I have this codex but I am not entirely sure from where - it may be from the Iokath Chapter or the Iokath daily area, it looks like it might be leaning more towards the daily area. There are two versions, one if you have Senya in your Alliance and one if you don't and Sana-Rae takes her place.
